PAY-PER-SERVICE OPTION
Below is a list of the most common uninsured services. If you choose Pay-Per-Service, you will be asked to pay for uninsured services prior to receiving them according to the following fee schedule recommended by the B.C. Medical Association.

Preventative services and screening tests not supported by evidence of medical effectiveness (for example, routine annual “complete” physical examinations, whole body CT scans, prostate specific antigen (PSA) tests).

Forms and certificates are uninsured when they are provided at the request of the patient and are transmitted to a third party such as an employer, private insurer, and others.

Telephone Advice and Prescription Renewals are uninsured when requested by the patient, and are not associated with an insured service, and no other insured service is provided.

Missed Appointment charges will apply unless the patient provides 24 hours notice.

Faxing & Photocopying is uninsured when it is provided for the patient’s personal use or for a third party.

Transfer of Medical Records is uninsured when it is at the request of the patient. Additional postage or faxing charges may apply.

Referral Notes are uninsured when the note is requested by the patient to obtain services that are not medically necessary.

Removal or treatment of warts is an insured service only for plantar warts (feet only), genital warts and all warts in immuno-compromised patients and children.

Travel advice for one destination is $60. If two destinations, $100. If three destinations, $125. This fee is for up to two people, per visit.

If you do not wish to pay for uninsured services one-by-one, sign up for the Extended Coverage plan that best suits your needs, and be covered for an entire year.

Individual Plan $240.00/year
Family Plan (Includes spouse & dependent children under 19) $360.00/year
The Extended Coverage plan covers all of the above uninsured services. Please review the services included as there has been some changes since last year.
